Naming Your Baby
Verification of birth questionnaire for newborns at Denver Health can now be submitted completely electronically during your hospital stay. The birth registrars work with newborn parents to compile all necessary information to send to the State of Colorado Public Health Department.
We will send you a questionnaire in the MyChart application, to complete prior to your discharge from the hospital. If you do not have an account, call us at 303-602-8000 and we can help set one up for you.
